I have a micro sd card on this phone , and one day i woke up ant the card requested a security code. i never set one on my card and if i try to format it it still demands the security code. what can i do?

i got a 6500s from almost 3 years ago...nice features! then i gave it to my son and w less than a year of use its flex show signs of tearing sounds when u opened/closed it..well i swapped it with a n73!
